Type 2 Diabetes Program

Our program
  • Individual assessment (45 minutes) with either an Accredited Exercise Physiologist (AEP) or Accredited Practicing Dietitian (APD). To determine suitability/safety for participation in group program and goal setting.
  • 6 group sessions with our AEP Jacynta, 2 group sessions with our APD Tess. Group sessions held once per week.
  • Individual review session with either AEP or APD (optional though strongly encouraged). To review measures taken at initial assessment and progress towards goals.
Groups will consist of 4 - 8 participants

What You Need
  • Referral from your GP if you are eligible to access Medicare funding or are over the age of 55.
  • Wear comfortable, non-restrictive clothing and footwear to each session. You will be required to be physically active in all sessions except group sessions 2 & 6 as they are discussion sessions with our dietitian.
  • We encourage you to have blood measurements taken, including HbA1c, within a month prior to commencing the program and again within a month after completing the 8 group sessions. We hope you see positive results!

Medicare funding for the program may be available to those diagnosed with Type 2 Diabetes Mellitus who meet the eligibility criteria as determined by their GP. An appropriate referral form is required to access any available funding. This funding is only available once per calendar year. Funding not available for individual review sessions. There is no gap fee charged for eligible participants.

The program is also suitable for those with Type 1 Diabetes Mellitus or those who may have been advised by their medical practitioner that they have "pre-diabetes” or have had "borderline” blood glucose measurements.

  • Group session (60 min) $20 each (discounts applicable if buying sessions in bulk)
  • Individual review assessment (30 min) $50

You can choose to continue attending sessions following completion of the initial program at the fees stated above. Please ensure that you book in for these sessions at least 24 hours in advance. We would advise that you take up the individual review sessions every 8-10 weeks, to ensure that your goals are being met, and new goals set as you continue to improve your health.
